2  Mail us the following documents:

a
Original, signed United States passport with at least 6 months of remaining validity.
b
Passport-type photographs: 2
c
Itinerary. Copy of round trip tickets or confirmed itinerary.
d
Yellow Fever Vaccination. Copy of International Certificate of Vaccination for Yellow Fever.
e
Bank Statement. Copy of a recent bank statement showing proof of sufficient funds.
Former Cameroonian citizens are required to turn in their old Cameroonian passport.

Additional required documents for minors (under 18):

- Birth Certificate. A copy of the applicant's birth certificate.
- Letter of Consent. A NOTARIZED letter of consent signed by both parents, specifying who will be traveling with the child and authorizing the Consulate to issue a visa to the child.
- Parents' Proof of Status. If the parents of the minor are of Cameroonian origin, the parents' ORIGINAL Cameroonian passports and copies of their US immigration documents (Green Card, I-20, etc) must be provided. If the parents are not of Cameroonian origin, a copy of the passport information pages must be provided.
- Third Party Proof of Status. If the minor is traveling with a third party of Cameroonian origin, the party's ORIGINAL Cameroonian passport and copies of his or her US immigration documents (Green Card, I-20, etc) must be provided. If the third party is not of Cameroonian origin, a copy of the passport information pages must be provided.

Yellow Fever Vaccination. Copy of International Certificate of Vaccination for Yellow Fever.

Business Letter. A business letter from the sending company in the US. The letter should be printed on the company letterhead stationery, addressed to "The Consulate of Cameroon, Visa Section", and signed by a senior manager (an equivalent to Vice-President or above). The business letter must adhere to the following guidelines:
• Briefly introduce the applicant (please specify employment status/position held in the company by applicant).
• State the nature of the business to be conducted (ie. business meetings, contract negotiations, etc.) and the names and addresses of companies to be contacted in Cameroon.
• Specify the type and desired validity of the visa (ie. a one year multiple entry business visa).
• Guarantee of sufficient funds for travel.

Former Cameroonian citizens are required to turn in their old Cameroonian passport.

Letter of Invitation. A certificate of accommodation "certificat d'hebergement" which is undersigned/NOTARIZED by Administrative or Immigration authorities in Cameroon. Note that this is not required if the applicant is of Cameroonian origin.
